Complaints of Discrimination or Harassment

Concerns about Negative Behavior and Campus Belonging

If you have a concern about behavior that negatively affect the climate of inclusion and belonging on campus, you can share your concern with the Campus Belonging staff in the Office of the Vice Chancellor for Diversity, Equity & Inclusion.

Reporting an Incident of Sex-Based Misconduct

Deciding to report sex-based misconduct and choosing how to proceed are difficult decisions. We recognize that your feelings about whether to report an incident may change over time. By using this online reporting form, you are submitting information to the University of Illinois Urbana-Champaign Title IX Office and the Office for Access & Equity.

Reporting a Barrier to Access

Barriers to access may include things such as inaccessible websites, videos without captions, a blocked access ramp, facility issues, or other lack of access to an event or program. Your response will be shared with the university’s ADA Coordinator.

Reporting Issues to the Title VI Office

Reports of Title VI Misconduct should be made directly to the Title VI Office. Any university office, unit, department, or college that receives a report of or observes an instance of Title VI Misconduct must report such to the Title VI Office.

Title VI Misconduct includes discrimination, harassment and related retaliation on the basis of race, color, and national origin, including shared ancestry and ethnic characteristics. To learn more about Title VI Misconduct, visit the definitions page.

University employees with supervisory or management responsibilities are obligated to report any and all observations and complaints of Title VI Misconduct to the Title VI Office.

Reporting a Violation of the Student Code

The Office for Student Conflict Resolution supports a safe and secure university environment that is conducive to academic success by administering the Student Code, promoting ethical decision making, encouraging personal and social responsibility, and facilitating the effective resolution of conflict.

Anyone who has information about a possible violation of the Student Code may report this information to the university by scheduling a meeting with a case coordinator. To do so, you may either call the office during our regular business hours (Monday through Friday, between 8:30 a.m. and 5 p.m.) or email conflictresolution@illinois.edu with your name, phone number, and a brief description of the behavior you would like to report.

Reporting to External Agencies

Anyone who believes they have experienced discrimination or harassment, including sex-based misconduct, at the University of Illinois has the right – and may initiate – a complaint with an appropriate state or federal compliance agency. Students and employees may file sex-based misconduct complaints with the Illinois Department of Human Rights (IDHR). Students may also file complaints with the U.S. Department of Education, Office for Civil Rights (OCR). In addition, any University employee may file a complaint with the U.S. Equal Employment Opportunity Commission (EEOC). Contact information for each agency appears below.
